Additional Information About Consents

Leaving One&All Hub Supervised

One&All’s programs of support may include activities that enable participants to leave the One&All Hub supervised during normal program hours, including for the purpose of walking to a local park.

Where you consent to a participant leaving the One&All Hub supervised during normal program hours, that consent is given on the basis that:

• the participant you will be accompanied by one or more of our staff and, where applicable, the participant’s support worker, and

• we will be responsible for the participant if she/he/they leave the One&All Hub supervised during normal program hours.

Leaving One&All Hub Unsupervised

The participant may wish to be able to leave the One&All Hub unsupervised during normal program hours, including for the purpose of:

• getting food or a coffee,

• returning home before a program has finished for the day, or

• removing herself/himself/themself from a situation that is making her/him/them anxious.

Where you consent to a participant leaving the One&All Hub unsupervised during normal program hours, that consent is given on the basis that:

• the participant:

  • is considered safe while unsupervised in the community,

  • before leaving the One&All Hub, will advise our staff that she/he/they are leaving, how long she/he/they anticipate being away from the One&All Hub and, where applicable, that she/he/they will not be returning to the One&All Hub, and

  • will, where applicable, return to the One&All Hub in a timely manner,

• we will not be responsible for the participant if she/he/they leave the One&All Hub unsupervised during normal program hours, and

• you have discussed the participant’s responsibilities with her/him/them and confirmed that she/he/they them.

Withdrawing or Altering Consents

You have the right to withdraw or alter your consents at any time by notifying us in writing at info@oneandallhub.org.
